I have a few questions regarding Clomid?
You should start using 50mg from day 3/4/5 of your cycle for 5 days, right? Is it absolutely necesarry to have scans until ovulation and then do a trigger? Or can you predict ovulation accurately and just go mad in the fertile period?
The reason I ask this is because my gynae is so busy that he sees this as a waste of time.
We only have a Medi-clinic in town and he is the only gynae (1 of only 2) I would trust enough to go to. The other one is ancient and the rest are GP's delivering healthy babies. Some don't even have sonar machines!!
Also, if you had this dx would you also insist on a Clomid/Femara cycle:
Age: 29
TTC for 2 yrs 6 months
PCOS and Insulin Resistant
History of prolonged vaginal bleeding after stopping birth control
Treated with Cyclocapron and Provera and then 6 months of Climen
and Metformin 500mg 3 x per day.
First pg: (May 2008) Bleeding since 6 wks then m/c at 9 weeks
Second pg: (Sept 2008) Bleeding from 7 wks until 16wks then m/c at 24 wks (Jan 2009)
Prolonged use of Cyclogest and Utrogestan during both pregnancies.
History after 2nd miscarriage:
1st Period - 6 weeks after labour - lasted 4 days
1 official cycle - 34 days
2nd official cycle - 31 days
3rd Official cycle - 36 days
4 Official cycle - non existent - now at CD42
Do any of you see any reason why I cannot use Clomid or Femara?
And which is the best to use?

My protocol and results were

25mg day 3 to 7 - result. 4 Follies, size barely mature
25mg day 4 to 6 - result. 2 Follies, 1 x 20mm, 1 x 16mm
Menopur on cycle 1 day 5 to 9
Menopur on cycle 2 day 7 to 9
No Menopur
I dunno if you should start on 50mg without scanning technology, would hate for you to fall preg with 6 babies.
Maybe 25mg and take it from there.
What does Dr Paula think?
Dr Paula is probably too busy with really scary diseases and stuff at the moment! I mean really what could be more important than helping me out???

Wow, I didn't know you could overstim so badly on 50mg clomid, because everywhere I read it says to start with 50mg. Now you're actually scaring me a bit....6 would be a bit too much, we are only aiming for at least five, but not six!!!
Anther Q: Why did you start the Menopur if the 25mg from day 4-6 seemed enough for 2 follies?

ReplyDeleteMandy my cycles look like yours off BCP, all over the place. Clomid is actually very safe in terms of multiple pregnancies, you do have an increased risk of twins, but not higher order multiples. the basic clomid cycle is 50mg daily from day 5 to day 9. This usually leads to ovulation on day 14, but it may happen from day 12 to day 19. You can use it blindly (ie without sonars) i did in fact do one such a cycle, but obviously with u/s surveillance you do have a higher + rate, plus you can also trigger the follie with ovidrel when it reaches 18mm, like with IVF. This is what we did when I fell preggies with miss D. The other good thing is that even if you don't fall pregnant you won't have these looong cycles, mine was 28 days every time on clomid. The most follies I ever made was 3 on clomid. (Also the other reason for scanning is to check that 50mg is enough, some people need 100mg). Look I think your options would be a) use clomid blindly and see what happens, b)make your gynae do the scans for you, or c) find someone who will. This is not high level scanning, one only needs to check both ovaries for a follicle and measure it. I would have done it for you with love if you lived here in Gomorra. In fact any gp should be able to help you. Any way you can see someone else?? I really think clomid could be very helpful. xoxo
The stoopid Dr put me on Menopur, (aka the drug from hell) on my first 2 cycles. Goodness alone knows why, I still maintain that's why I didn't get BFPs on those 2. I was also triggered with Pregnyl and like Paula said, my cycle was exactly 28 days.
ReplyDeletePull out the confessional box... I didn't inject myself with Menopur on my last cycle, told them I did, but didn't! I just couldn't shake the feeling that something was amiss with the combo.
Oh the other difference was the Pregynova (spelling Dr Paula)?? which I took from CD10 until 12 weeks preg. The Clomid thinned my lining something fierce and I was given that to make it all fat and baby inviting.
